By signing up for a class or event, you are giving CDM permission for photos and videos to be taken and used (children's names will NOT be used), however, you may opt out by filling out the Media Consent form.

Liability Release

When signing up for a class/event online, this was ALREADY agreed to and signed; you do not need to fill it out again. However, for visitors, one-time participants, volunteers, sign-ups not through the website, etc., this form signature is REQUIRED before participating.
